Name: Address: City, State Zip: Email address: Phone number: USBC # for average verification (certification not required to bowl) (If no average is published must submit proof of average from a league secretary, league sheets, or from a tournament director NO HANDWRITTEN verification accepted must be typed or printed on letterhead and have telephone contact information.) This average MAY be accepted but Director has the right to assign any average to any bowler at any time. I do hereby state that I have never won a PBA National or Regional Title as a member or guest, been a member of Team USA, won an Eagle at the USBC Men’s or Women’s divisions, ABC, or WIBC Championships, all the bowlers that fit into the above categories will bowl scratch regardless of current USBC average(s) reported to Bowl.com. All international bowlers will bowl at 210 until they have established a 12-game average in Bowl Florida and then will be subject to all other average restrictions as put forth in the rules. Tournament Director has the right to assign any average at any time to any bowler. Current league average: Bowling center name(s): Highest average past 2 yrs: I agree and attest that the above information is correct to the best of my knowledge and I agree to abide by all the rules of BOWLFLORIDA.COM. I also understand that if I violate these rules I can be removed from the competition and disqualified from participating in any future events with no refund of any monies paid.
